White Paper: The promise and potential of the CHRONIC Care Act

Health care providers have increasingly focused on value-based care for several years, with many hospitals investing billions to modernize their data systems. However, few, if any, of these investments were driven, or even informed, by the need to coordinate non-medical care outside of clinical settings.

The CHRONIC Care Act (CCA), which became law in 2018, provides a timely vehicle to facilitate this change. Driven by patients, physicians, advocacy groups, and various health care professionals, the CCA represents a potentially seismic shift in how Medicare Advantage (MA) plans can coordinate care for their members.

Read this white paper for:
  • A summary of the changes coming to human and social services
  • A view into some of the changes facilitated by the CHRONIC Care Act
  • A series of questions that state policy makers need to consider as MA plans begin to coordinate more supplemental, non-medical benefits
